I. Student registration procedures
- Regarding leaves of absence or withdrawal -
Documents to be submitted: Leave of Absence or Withdrawal Request Forms
For leaves of absence (if you are unable to come to school for a period of 3 months or longer) or for withdrawals from school, please consult with your guarantor and your supervisor, and file requests at least one month before the desired date.
* If you need a Leave of Absence or Withdrawal Request Form, you can receive them from the School of Health Science Administration Team Education Contact. - About returning to school -
Documents to be submitted: Re-enrollment request form
Requests must be made at least one month before the leave of absence period expires.
(If you are taking a leave of absence due to illness, please include a health certificate from your doctor.)
* If you wish to return to school, you can receive the forms needed from the School of Health Science Administration Team Education Contact. - Other
If changes occur due to any of the following reasons, please file the necessary forms.
