I. Student registration procedures

  1. Regarding leaves of absence or withdrawal -
    Documents to be submitted: Leave of Absence or Withdrawal Request Forms
    For leaves of absence (if you are unable to come to school for a period of 3 months or longer) or for withdrawals from school, please consult with your guarantor and your supervisor, and file requests at least one month before the desired date.
    * If you need a Leave of Absence or Withdrawal Request Form, you can receive them from the School of Health Science Administration Team Education Contact.
  2. About returning to school -
    Documents to be submitted: Re-enrollment request form
    Requests must be made at least one month before the leave of absence period expires.
    (If you are taking a leave of absence due to illness, please include a health certificate from your doctor.)
    * If you wish to return to school, you can receive the forms needed from the School of Health Science Administration Team Education Contact.
  3. Other
    If changes occur due to any of the following reasons, please file the necessary forms.
